Forms

Here you find the following forms, which you need, if you want to use the corresponding services provided by the group IT:

	Conditions of Use	Conditions of Use for Information-Processing Systems at DESY
User Account:	User Registration Form	s. auch User Accounts on Central Systems at DESY
Grid Certificate:	GRID Certificates and VO Membership	You have to hold a personal user certificate if you want to access the NAF (naf.desy.de) or the global GRID resources. For the GRID you have to be additionally member of a Virtual Organization (VO).
RAS - Modem/IDSN: RAS -VPN:	User Registration Form	On a registration form, which has to be signed by your Namespace Supervisor, please choose 'Other Machines and Services' and mark 'RAS - Modem/ISDN' or 'RAS VPN'.
Modem Account with callback:	Modem Form	This form has to be signed by your Deputy Director/Bereichsreferent.
Additional Security Commitment concerning RAS-Accounts:	RAS Letter of Commitment	This form has to be signed by the Namespace Supervisor and the user applying for a RAS-Account for access via VPN, Modem or ISDN.
Software:	Borrowing slip for software	You need it, if you want to install software, which you cannot get centrally over the DESY network. For more information see Services -> Software -> (Other Opportunities)
WLAN:	Borrowing slip for WLAN-Card	Here you can find information about wireless access to the DESY (guest) network, for which you need such a card. You can borrow it for a limited time.
System Registration	System Registration Form	Here you can find information about the connection of individually administered systems to the DESY Intranet
Webserver Registration	Registration Form for webservers with internet access	Is necessary for the approval of an internet accessible webserver.
